For-Profit Organization - Pay Online

A KOHC For-Profit Organization Membership is $500 annually, and includes one vote per organization and 12 named members. Additional named members may be added for $25 each.

Each named member is invited to participate in educational and networking opportunities during quarterly and annual meetings. The voting member or their proxy may vote on KOHC legislative priorities, election of officers, and other coalition decisions at the annual meeting in December.

Dental CEUs are available to all named members for KOHC quarterly and annual meetings.

Thank you for submitting your Small Business and primary member contact information. A total of 3 named members are included in your $100 Small Business membership. Additional named members can be added to your membership for $25 each, up to 10 total.

After you click the "JOIN" button at the end of this form, you will view a confirmation page and receive an email that will include a link to the Named Member Form. Please send the link to anyone you would like to include as a named member on your membership so that their information is added to your account in our records. Only named members have the opportunity to earn dental CEU credits for attendance at KOHC quarterly and annual meetings.
