33rd Ward Direct Cash Assitance provides financial assistance to households that were not eligible to receive one or more Economic Impact Payments (EIP) from the Internal Revenue Services (IRS) due to immigration status.

-Financial Assistance Amounts-
• $1,000 per household

In order to be eligible, you must:
1. Have been an 33ward resident for three months
2. Have not received ONE or MORE Economic Impact Payments (EIP) from the Internal Revenue Services (IRS), i.e. Stimulus Checks
3. Income does not exceed 80% of Area Median Income (Calculator included later in this form)

While priority will be given to those who have not received any cash assistance from other programs, please apply nonetheless if you are eligible.

Please have the following documents prepared:

One Proof of Identification: (State ID, Driver's License, Temporary Visitors Driver's License [TVDL], Chicago City Key, Matricula Consular)

For children One: (Birth certificate, Passport, School ID, Matricula Consular)

One Proof of Address: (State ID, Driver's License, TVDL, Utility or Phone Bill, Lease or rental agreement, Mortgage Statement, Bank or Credit Card Statement, Paystub)

One Proof of Income: (Letter from Employer, Most recently filed taxes or W-2, Most recent Paystub. If none, Signed affidavit of income)

Si bien se dará prioridad a aquellos que no hayan recibido asistencia en efectivo de otros programas, presente su solicitud si es elegible.
