Research shows that Black and Hispanic/Latino people are more likely to have worse health outcomes, shorter lives, and higher rates of chronic conditions (such as diabetes, asthma, or hypertension). These are known as health disparities. Communities of color often times do not have good access to things like health insurance, transportation, healthy food, safe spaces to exercise, and quality, affordable housing. These are known as health inequities. If you have ever experienced health disparities or health inequities, please fill out the brief survey below. TJC wants to share your experiences to educate people about these issues and get them to take action to fix the problems in our healthcare system. If you select the box indicating that you would like to remain anonymous, your name will not be shared with your responses.
