Our Dementia Values & Priorities Tool will help you think about and document your wishes regarding the type of care you want if or when you are living with advanced dementia. We’ll walk you through this new tool to share how we are working to transform how people live and die with dementia to ensure they are empowered and supported in getting the care they want, or avoid the care they don’t want.
