Washington Faculty Forward is part of a national movement to fight the crisis in higher education.  I hereby voluntarily request and accept membership in SEIU Local 925 and agree to abide by its Constitution and Bylaws and by the Service Employees International Union Constitution and Bylaws. This membership authorization is to remain in full force and effect until SEIU Local 925 has received written notification from me of its termination in such time and in such manner as to afford SEIU Local 925 a reasonable opportunity to act on it.
I voluntarily authorize SEIU 925 to make withdrawals from my checking or savings account or to bill my credit card account as indicated below, in accordance with the authorization provided below. In the case of checking and savings accounts, adjusting entries to correct errors are also authorized. I agree that these withdrawals and adjustments may be made electronically and under the Rules of the National Automated Clearing House Association. I may cancel this dues payment authorization. Cancellation of payment authorization may be made by a permitted method, including electronically.
Contributions or gifts to SEIU Local 925 are not tax deductible as charitable contributions for federal income tax purposes. However, they may be tax deductible as ordinary and necessary business expenses.
