I hereby grant Mi Familia Vota and its employees and independent contractors the irrevocable right to use my image, voice and name in photographs, videotapes and audio recordings in connection with any event hosted, co-hosted, and/or promoted by Mi Familia en Acción. I understand that this use may include publications and distribution in printed, electronic and digital media, including, but not limited to, course materials and brochures, videotape and television programs, and website, social media and online communications.

I hereby release, waive and discharge Mi Familia en Acción and its trustees, employees, and independent contractors from any and all demands, claims, causes of action, damages and liabilities directly or indirectly arising out of any use of my image, voice or name.
