To schedule an event, please fill out the following form with as many details as you can.

Once submitted, a member of the TACA team will review and approve your event and you will receive a notification. Should there be any questions that we'll need to address before approving your event, a TACA team member will reach out to clarify details as needed before approving your event.

Terms & Conditions

The undersigned and any minor children for whom I have the capacity to contract for; voluntarily agree to participate in the event sponsored by The Autism Community in Action (TACA).

The undersigned recognizes that TACA has not undertaken any duty or responsibility on behalf of myself and any minor children for whom I have responsibility; for our safety and the undersigned agrees to assume the full responsibility for all risk of bodily injury, death, disability, and property damage as a result of participating in this event.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. TACA has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19 but cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other guests and their families. I am voluntarily attending this event and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending the event.

By my signature, I hereby release and agree to hold The Autism Community in Action harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the organization, or that may otherwise arise in any way in connection with attending The Autism Community in Action (TACA) event. I understand that this release discharges The Autism Community in Acton (TACA) from any liability or claim that I, my heirs, or any personal representatives may have against the organization and its directors, officers, employees, volunteers, and other agents with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, attending The Autism Community in Action (TACA) event.

I give permission to TACA to take photographs of me, my children, and any minor children whom I have the capacity to contract while participating in this activity for use in future publicity and understand that I will not receive any compensation for such use.
