By completing this form I hereby state that I am the parent or legal guardian of the student I have herein registered for the Marquette Catholic High School (MCHS) summer camps and hereby authorize the coaches and camp staff to act on my behalf, using their best judgement, in any situation where medical attention may be required for my son or daughter. I know of no mental or physical conditions which may effect my child's ability to safely participate in this camp (s). I understand and agree I am responsible for all fees associated with attendance at this camp, including medical expenses which may be required. I hereby agree to release and hold harmless all MCHS coaches, staff, volunteers, administrators and similar personnel from any claims which may hereafter be presented on behalf of my child for any such injuries which may occur from participation in this camp.

I hereby authorize the coaches of Marquette Catholic High School to act for me using their best judgment in any situation requiring any medical attention. I know of no mental or physical conditions which might affect my child's ability to safely participate in this camp(s). I agree to assume any expense medical or otherwise associated with attendance to this camp(s). I also agree to release the coaches and trainer of Marquette Catholic High School from any such claim, which may hereafter be presented by my child as a result of such injuries.
